We are a countywide community specialist team which provides assessment for Gloucestershire residents who have suspected heart failure, providing heart scans called echocardiograms. 

If the scan shows that a patient does have heart failure, they are given an appointment to be reviewed and assessed by a General Practitioner with a Special Interest (GPwSI) in heart failure and a heart failure specialist nurse who, with the patient, agree a treatment and management plan.

Our heart failure specialist nurse team also manages patients who have had a recent admission to hospital for heart failure.

Patients are referred into our service by their GP and we do not accept self-referrals. Please consult with your GP if you wish to access the service.

Patients and carers have access to a helpline staffed 9.00am – 5.00pm Monday to Friday.

Q. What is heart failure?
A. The term ‘heart failure’ is used to describe the signs and symptoms which can happen when the heart muscle is unable to pump as efficiently as it should.

Q. How common is heart failure?
A. Heart failure is very common and although not curable there are many very effective treatments that are available to improve symptoms, improve quality of life and help people live longer.

Q. What is an Echocardiogram?
A. An echocardiogram or ‘echo’ is an ultrasound scan of your heart.  The machine will be able to show us pictures of your heart and how well it is pumping.
