Since November 2013 the Trust has been introducing SystmOne, an electronic clinical recording system, across several countywide services.

SystmOne helps the way we keep your health records up-to-date, as well as sharing them with other clinicians and healthcare professionals. It has enabled many of our clinicians to record electronically in the homes of service users - so if you are visited by a member of our Integrated Community Teams (ICT), they will be recording your information onto a laptop. And if you are in one of our community hospitals or Minor Injury or Illness Units (MIIUs), clinicians will be using Workstation on Wheels (WOW), rather than a paper and pen, to electronically document any care provided to you on your health record.

All this means your information can be shared electronically with other clinicians providing care for you. They are able to access the most up-to-date information and we can better understand your care needs. You can view your healthcare record whenever you are receiving care from one of our services - just ask the clinician!

For those receiving care at home, it is important to provide information in your home so that all involved in your care are kept informed, including family. So even with the move to electronic records, a communication journal (paper record) will be kept in your home. It also means that you and your family can record questions or concerns that can then be dealt with by your carers.
