We are the new NHS Trust responsible for providing high quality community health and adult social care services in Gloucestershire.
Formed as an NHS Trust in April 2013, we provide a wide range of health and social care services for people of all ages – from school nursing to physiotherapy, podiatry to cardiac nursing, adult social care to telecare – as well as managing the county’s seven community hospitals.
We're a people organisation, employing 2,800 highly-skilled staff and managing a further 800 adult social care staff on behalf of Gloucestershire County Council, and working closely with other health professionals to ensure a co-ordinated approach to care.
Together, we work hard to make sure that our patients and service users receive the right support, from the right people, at the right time.
Community hospitals play a vital role in caring for patients of all ages.
Working closely with local GPs and Gloucestershire Hospitals NHS Foundation Trust, we provide in-patient care as well as a range of out-patient clinics and services, including Minor Injury Units.
Our seven community hospitals provide high quality care that is centred on the needs of patients and their families, provided by our skilled and dedicated staff.
Major investments in our hospital buildings, new ways of working with partner organisations and a focus on continuous improvement means we are well-placed to offer local people the highest quality of care, close to home.
Bringing care closer to home
We aim to provide local people with the highest quality health and social care, close to or within their home.
By bringing together teams which include occupational therapists, social workers, physiotherapists, community nurses, reablement workers and with GPs, we aim to provide care at home or close to home for our community. We will also ensure we refer on for specialist care when it is needed.
Together, we support people to be in control of their choices and to maintain their independence safely and appropriately.
We provide a wide range of high quality, community health and adult social care services for people of all ages.
In local communities acrosss Gloucestershire, we provide a range of services, such as district nursing, health visiting, school nursing as well as specialist services such as sexual health, cardiac care, IV therapies, telehealth, dentistry and many more - as well as our community hospitals. We also deliver adult social care services in partnership with Gloucestershire County Council.
